Transitional Care Clinic at UNC Health Blue Ridge

The Transitional Care Clinic (TCC) is a new service within Blue Ridge Medical Group designed to support patients during the important first weeks after leaving the hospital. Our goal is to make sure every patient has a safe, smooth transition back home with the follow up care, education, and resources needed to stay well.

The TCC brings together medical care, care coordination, and patient support in one place. The clinic works closely with inpatient teams, primary care providers, virtual care programs, and community partners to reduce readmissions and help patients feel confident managing their health.

How the Transitional Care Clinic Helps Patients

Patients seen in the TCC receive:

  • A timely follow up visit after hospital discharge
  • A coordinated care plan tailored to their health needs
  • Education on medications, symptoms, and self care
  • Support navigating appointments, resources, and services
  • Connections to community programs and long term providers
  • A short term “bridge” until they transition back to their primary care home

Who We Serve

The TCC is designed for patients who may benefit from extra support after leaving the hospital, including those with:

  • Complex medical conditions
  • Multiple medications
  • Recent hospital admissions
  • Social or economic barriers that make care harder to manage
  • Limited access to transportation or health resources
  • Difficulty connecting with a primary care provider

Meet Our Physician Leader

Dr. Paul Fletcher is board-certified in Internal Medicine and Pediatrics. He is a graduate of the West Virginia School of Osteopathic Medicine, Lewisburg, and completed his residency at Penn State’s Milton S. Hershey Medical Center.

Dr. Fletcher is committed to reducing readmissions and improving continuity of care.
